UHS Transcript Request & International Evaluation Agencies

To request for your Academic transcript from Ultimate Health School,

If you are requesting for more than one Academic transcript, you will have to fill out the transcript

 

request form per request.

 

The cost for each transcript request is $20.00

 

Student must allow a maximum turnaround time of seven school days for the request to process.

 

If you are a new applicant to Ultimate Health School but your educational transcript originates from an institution outside the United States, you can use one of the International transcript evaluation agencies listed on this website to evaluate your academic transcript.
